Comhar Inc. is looking for a Bilingual Blended Case Manager Supervisor (Spanish Speaking) to join our team.

As a Bilingual Blended Case Manager Supervisor, you will oversee Teams of Blended Case Managers, ensuring the delivery of high-quality case management services to adults with serious mental health diagnoses. You will be responsible for maintaining staff knowledge of Standards of Practice, providing daily supervision, conducting chart reviews, and monitoring productivity. Additionally, you will assist individuals in developing goals for stability and meaningful living.

Responsibilities

• Provide on-site supervision to Teams of Blended Case Managers.
• Maintain current knowledge of the Standards of Practice for Blended Case Management.
• Assist individuals in developing goals for stability and meaningful living.
• Monitor staff productivity and ensure documentation meets guidelines.

Requirements

• Master's Level degree in social work, psychology, or related field.
• Three years of mental health direct care experience, including supervisory experience.
• Knowledge of Blended Case Management Standards of Care.
• Ability to work effectively in a fast-paced environment.
• Bilingual in Spanish and English.
