Home Health Registered Nurse (RN)

Job Description

We are looking for a compassionate Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach, serving Vero Beach, Ft. Pierce, and Port St. Lucie, Florida. Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Collaborate with physicians and supervise LPNs, CNAs, and aides in delivering patient care.
• Conduct patient assessments, document progress, and communicate changes to the healthcare team.
• Educate patients and their families on care plans, medications, and wellness activities.
• Ensure care goals are met and facilitate patient discharge when appropriate.

Requirements

• Minimum 1 year of acute care experience; home healthcare experience and Oasis familiarity preferred.
• Graduate of an accredited nursing program with an active RN license in Florida.
• CPR certification and knowledge of state and federal home health regulations.
• Strong clinical skills, communication abilities, and empathy for patients.
