Job Description
We are looking for a dedicated Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach. Responsibilities
- Provide home-based care to patients with illnesses, injuries, and chronic diseases, focusing on post-discharge assistance.
- Collaborate with physicians and supervise LPNs, CNAs, and aides involved in patient care.
- Conduct patient assessments, document progress, and communicate changes to the treating physician.
- Offer patient education on care plans, prescribed medications, therapies, diets, and exercise routines.
- Educate family members on patient care plans, medications, and home safety measures.
- Ensure treatment goals are achieved and manage patient discharge processes.
Requirements
- Minimum of 1 year of acute care experience, preferably with home healthcare experience.
- Experience with Oasis is advantageous.
- Graduate of an accredited nursing program with an active RN license in Florida.
- CPR certification is required.
- Familiarity with state and federal home health regulations.
- Clinical proficiency in a nursing specialty, excellent communication skills, and a compassionate approach.
