Job Description
We are looking for a compassionate Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach. Responsibilities
- Provide home-based care to patients post-hospitalization or rehabilitation.
- Collaborate with physicians and oversee care providers.
- Conduct patient assessments, document progress, and communicate changes to the medical team.
- Educate patients and their families on care plans, medications, and safety measures.
- Ensure treatment goals are achieved and assist in coordinating discharge.
Requirements
- Minimum of 1 year acute care experience.
- Home healthcare and Oasis experience preferred.
- Active RN license in FL and CPR certification.
- Familiarity with state and federal home health regulations.
- Strong clinical skills, communication abilities, and a compassionate approach.
