We are looking for a dedicated Home Health Registered Nurse (RN) to join our team in Port St. Lucie and Okeechobee, FL. Responsibilities:

• Provide home-based care to patients post-hospitalization
• Collaborate with physicians and oversee patient care team
• Conduct patient assessments, track progress, and report changes
• Educate patients and families on care plans
• Ensure care goals are met and coordinate discharge

Requirements:

• 1 year of acute care experience
• Active RN license in FL
• CPR certification
• Familiarity with home health regulations
• Excellent clinical skills and compassionate nature
