Responsibilities

• Deliver home-based care to patients recovering from illnesses, injuries, and chronic conditions.
• Collaborate with a team of healthcare professionals to ensure comprehensive patient care.
• Conduct patient assessments, monitor progress, and communicate changes to physicians.
• Educate patients and families on treatment plans, medications, and home care practices.
• Facilitate goal achievement and coordinate patient discharge when appropriate.

Requirements

• Minimum 1 year of acute care experience required; home healthcare experience is preferred.
• Active RN license in FL and CPR certification.
• In-depth knowledge of state and federal home health regulations.
• Strong clinical skills, effective communication, and compassionate patient care.
