Responsibilities

• Evaluate and treat patients, establishing outcomes-centered plans.
• Deliver effective care management across a patient's episode to ensure timely, progressive, and high-valued care.
• Operate at the top of your license, leveraging and delegating care amongst the clinic care team.
• Drive toward consistent and predictably positive outcomes for patients.

Qualifications

• Must be licensed or license eligible in the state of application.
