Job Description
- Explains, interprets, and coordinates patients' rights and advanced directives.
- Performs ongoing physical and psychosocial assessment of Hospice patients.
- Collaborates with IDT regarding new nursing diagnoses and care plan changes.
- Instructs and supervises hospice aides/LPN staff.
- Assists in obtaining durable medical equipment and essential supplies for patient care.
- Observes and records signs and symptoms of the disease process.
- Communicates with the physician, family, and health team personnel to report changes in the patient's condition.
- Facilitates patient and family self-direction and independence.
- Prepares and administers medications according to policy and documents the same.
- Provides support and relief for the primary care person and participates in 'at-home respite' program.
- Completes chart documentation within 48 hours.
- Maintains a caseload with productivity in compliance with agency standards.
- Rotates through the on-call system as scheduled for revisits.
- Incorporates patients' physiological and developmental age into the overall care plan.
- Maintains appropriate infection control practices.
- Participates in all required in-service education programs and meetings.
- Demonstrates compliance with the dress code policy.
Required Skills/Abilities
- Strong nursing assessment and documentation skills.
- Clinical knowledge to support the dying process.
- Excellent interpersonal and communication skills.
- Ability to comprehend the spoken and written English language.
Education and Experience
Required Education: Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred. Licensed to practice as a Registered Nurse in the State of New Jersey; licensure in Pennsylvania if practicing in PA.
Preferred Education: CPR certification preferred.
Required Experience: Minimum one-year adult, pediatric, or medical/surgical clinical experience.
Preferred Experience: Prior home care or hospice experience preferred.
