SOCIAL WORKER

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a vital part of our organization, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues related to institutional living, financial concerns, family dynamics, medical understanding, aging, discharge planning, and end-of-life support.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services to patients and families.
• Address issues related to adjustment, family relationships, financial concerns, and end-of-life care.
• Collaborate with healthcare professionals to ensure holistic patient care.
• Participate in discharge planning and provide counseling support.

Requirements

• Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
• Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
