SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and families, addressing a wide range of common issues such as adjustment difficulties, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on end-of-life matters.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families in coping with medical conditions and emotional challenges.
• Participate in discharge planning and provide counseling related to death and dying.
• Collaborate with healthcare teams to deliver comprehensive patient care.

Requirements

• Bachelor's in Social Work (BSW) is required; Master's degree is preferred.
• Licensed Certified Social Worker (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
