SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As part of the Allied Health team, you will provide clinical social work services to patients/residents and families, addressing various common problems and facilitating emotional support in challenging situations.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients/residents and families.
• Assist individuals in coping with institutional living adjustments, financial concerns, and family relationships.
• Support individuals facing medical challenges, aging-related changes, and end-of-life issues.
• Collaborate with healthcare teams on discharge planning and holistic care.

Requirements

• Bachelor's in Social Work (BSW); Master's degree preferred.
• LCSW-C certification is preferred; LGSW certification is required.
• Compassionate and empathetic approach to patient care.
• Strong communication and interpersonal skills.
