Job Description
LifeBridge Health is seeking a compassionate and dedicated Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a wide range of common issues.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist patients with adjustment to institutional living and aging-related changes.
- Offer support with financial concerns and family relationships.
- Participate in discharge planning and end-of-life counseling.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
