Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community.
• Implement patient-centered care plans using available resources.
• Participate in discharge planning for patients.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.

Job Details

• Job Types: Per Diem, Part-time, Full-time.
• Pay: $22.00 - $28.00 per hour.
• Expected Hours: 20 - 40 per week.
• Weekly Schedule: 5x8, Monday to Friday, Rotating weekends.
• Work Location: In person.
