Job Description
We are looking for experienced Home Care LPNs to join ChristianaCare's HomeHealth team. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to discharge planning. This position offers the opportunity to work full-time, part-time, or per diem, with competitive pay based on experience.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
