Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
