Job Description
We are seeking experienced Home Care LPNs to join our team at ChristianaCare. As a Home Health Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in patient discharge planning. Our Full-Time, Part-Time, and Per Diem positions offer flexible scheduling and attractive compensation packages.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans and participate in discharge planning.
- Support New Castle County, DE; work day or night shifts, one weekend a month, and 2 holidays a year.
Requirements
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.
Job Types & Benefits
We offer Per Diem, Part-Time, and Full-Time positions with competitive pay ranging from $22.00 to $28.00 per hour. Benefits include medical, dental, vision, 401(k), flexible schedule, and more.
Medical Specialties & Shifts

- Standard shifts: Day shift, Night shift.
Schedule & Location
Expect 20-40 hours per week with a schedule of 5x8, Monday to Friday, and rotating weekends. Work location is in person.
