Job Description
We are looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our team at ChristianaCare HomeHealth. As a Full Time, Part Time, or Per Diem LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
