Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in a community setting.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
