Job Description
We are looking for an experienced Home Care LPN to join our supportive team at ChristianaCare. As a Home Care LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
