Job Description
ChristianaCare HomeHealth is currently looking for experienced and dedicated Home Care LPNs to join our team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, helping them achieve their health goals.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented care plans that are patient-centered.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

Weekly Schedule
- Expected hours: 20 – 40 per week.
- Standard shift options: Day shift and night shift.
