Licensed Practical Nurse (LPN)

Job Description

Responsibilities

• Deliver and coordinate quality patient care following physician orders.
• Observe and assess patient conditions, reporting changes to supervisors.
• Document care activities and nursing actions accurately and promptly.
• Participate in updating nursing care plans and achieving expected outcomes.
• Educate patients and families on disease management and prevention strategies.

Requirements

• Graduate of an accredited nursing school.
• Current state LPN license and CPR certification.
• Proficiency in clinical assessments, documentation, and compliance with care policies.
• State-specific experience requirements may apply.

Preferences

• Recent clinical care experience or home health experience.

Skills/Abilities

• Attention to detail, time management, problem-solving, and communication skills.

Physical Requirements

• Ability to communicate in English, travel, lift 50 pounds, and perform various physical tasks.
