Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School diploma (HSD) or GED.
- Current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
