Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. As an LPN in our HomeHealth department, you will be providing skilled nursing care to geriatric patients in the community, implementing care plans, and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
