Job Description
We are looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at ChristianaCare HomeHealth. As an LPN, you will provide skilled nursing care and treatments to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
