Job Description
We are seeking dedicated Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Our state-of-the-art Electronic Health Record system and comprehensive training ensure that you excel in your role.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans.
Requirements
- High School Diploma (HSD) or GED required.
- Must hold a current DE LPN license or compact state license.
