Job Description
We are looking for a Licensed Practical Nurse (LPN) MDS Coordinator to join Genesis Health, a leading healthcare services provider committed to delivering high-quality care with compassion. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure exceptional patient care and proper reimbursement.
Position Highlights
- Lead the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for completion and electronic transmission of the MDS.
- Develop individualized resident Care Plans in accordance with Federal and State regulations, identifying resident needs and projected outcomes.
- Provide training on clinical content and electronic completion of MDS items to new team members.
Requirements
- Active LPN license in good standing.
- Prior experience in a healthcare facility, with knowledge of nursing practices and regulations.
- Proficient in computer applications, including Windows. Experience with AccuMed is a plus.
- Previous MDS Coordinator experience preferred, with demonstrated proficiency in RAI/PPS processes.
- Current BLS/CPR certificate required.
