Job Description

We are seeking experienced Licensed Practical Nurses (LPNs) to join ChristianaCare's HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans and contributing to discharge planning. Join a supportive team with a Class 5 rated Electronic Health Record and comprehensive OASIS-E training.

Responsibilities

• Provide skilled nursing care and treatment to geriatric patients.
• Implement goal-oriented, patient-centered care plans.
• Participate in developing discharge plans.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
