We are looking for experienced Home Health Licensed Practical Nurses (LPNs) to join ChristianaCare's HomeHealth team. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients within the community.
- Implement goal-oriented and patient-centered care plans.
- Assist in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
