Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN in our Home Health department, you will provide skilled nursing care to geriatric patients in the community. Duties and Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.

Job Details
Job Types: Per diem, Part-time, Full-time
Pay: $22.00 - $28.00 per hour
Expected hours: 20 – 40 per week
Medical Specialties: Geriatrics, Home Health, Pediatrics, Wound Care
Standard Shifts: Day shift, Night Shift
Supplemental Pay Types: Differential pay
Supplemental Schedule: Holidays, Overtime
Weekly Schedule: 5x8, Monday to Friday, Rotating Weekends
