Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN in our Home Care program, you will provide skilled nursing care and treatment to geriatric patients in the community. The successful candidates will play a crucial role in implementing patient-centered care plans and ensuring high-quality care services.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
