Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join ChristianaCare's HomeHealth team. If you are passionate about providing skilled nursing care to geriatric patients, implementing patient-centered care plans, and contributing to discharge planning, this role is for you.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
