Job Description
ChristianaCare HomeHealth is currently looking for dedicated Licensed Practical Nurses (LPNs) to join our team. If you are experienced in HomeHealth care and want to contribute to a supportive and technologically advanced environment, this opportunity is for you.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Develop and implement patient-centered care plans in collaboration with the healthcare team.
- Participate in discharge planning for patients under your care.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
