LPN MDS Coordinator Position at Genesis Health

We are currently seeking an experienced LPN (Licensed Practical Nurse) to join our team as an MDS (Minimum Data Set) Coordinator. As part of Genesis HealthCare, you will play a crucial role in the Resident Assessment Process, ensuring high-quality patient care and accurate reimbursement for services provided.

Position Highlights

• Direct the interdisciplinary RAI (Resident Assessment Instrument) process.
• Develop individualized resident Care Plans based on resident needs and regulatory requirements.
• Provide training on clinical content and MDS completion to new team members.

Requirements

• LPN license in good standing.
• Prior experience in a healthcare setting.
• Knowledge of nursing practices and regulations.
• Computer proficiency, including Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification required.
