Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a motivated Minimal Data Set Coordinator who holds a Licensed Practical Nurse (LPN) certification to join Genesis HealthCare, a leading healthcare services provider. As the LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and appropriate nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in compliance with guidelines for MDS completion, encoding, and electronic State transmission.
• Contribute to developing personalized resident Care Plans, identifying resident needs, and expected outcomes as per Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital, long-term care, or healthcare-related facilities.
• Proficient in general, rehabilitative, and restorative nursing practices.
• Familiarity with laws, regulations, and guidelines in long-term care.
• Computer literacy with Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• Certification in BLS/CPR is required.
