Minimal Data Set Coordinator - LPN

Job Description

Genesis Health is looking for a dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse (LPN) to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in the nursing center, adhering to guidelines for MDS completion and transmission.
• Contribute to the development of individualized resident Care Plans, focusing on resident needs and expected outcomes as mandated by Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in hospitals, long-term care facilities, or similar healthcare settings.
• Familiarity with general, rehabilitative, and restorative nursing, medical practices, and relevant laws and regulations.
• Proficiency in Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience preferred.
• Competency in RAI/PPS processes.
• Current BLS/CPR certification.
