Job Description: Nurse-LPN MDS Coordinator

In this role, you will participate in the Resident Assessment Process by completing items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans.
• Train team members on clinical content and electronic MDS completion.

Requirements

• Current LPN license.
• Prior experience in healthcare facilities.
• Proficiency in RAI/PPS processes.
• Computer skills, especially with windows applications.
