Physical Therapist - $70-$90 per Hour

Paradigm Rehab is seeking a compassionate and dedicated Physical Therapist to join our team and make a difference in the lives of our patients. As a Physical Therapist, you will conduct assessments and provide direct patient care to enhance functional capabilities and promote rehabilitation.

Responsibilities

• Conduct home visits to assess patient's medical conditions and limitations.
• Document patient information accurately using EMR system.
• Collaborate with interdisciplinary teams to develop tailored treatment plans.
• Ensure patient's safety and well-being during therapy sessions.

Requirements

• Graduate from an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• CPR Certification.
• Experience with EMR documentation is a plus.
