Physical Therapist Responsibilities:

• Conduct home visit assessments to evaluate patients' medical conditions and functional capabilities.
• Deliver direct patient care and determine potential for rehabilitation.
• Proficient in EMR documentation (experience in Hello Note is a plus).
• Maintain an unencumbered Physical Therapy license for New York.
• Be CPR certified.
