Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and functional abilities.
• Provide direct patient care and develop rehabilitation plans.
• Ensure accurate EMR documentation.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficiency in CPR and EMR documentation is preferred.
