Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visits to perform assessments and provide direct patient care.
• Proficient in EMR documentation.
• Hold a valid CPR Certification.
