Job Description

We are seeking a dedicated and experienced Registered Nurse (RN) Field Nurse specializing in Home Care to join our team at ChristianaCare. As a valuable member of our healthcare organization, you will provide skilled nursing care to geriatric patients in the community, develop and implement care plans, and ensure exceptional patient-centered services.

Responsibilities

• Deliver skilled nursing care and prescribed treatment to geriatric patients in a home care setting.
• Develop patient-centered care plans and goals consistent with available resources.
• Create discharge plans for patients based on individual needs.
• Maintain IV proficiency or undergo training to effectively perform IV functions.

Requirements

• BSN required, ADN candidates must obtain BSN within three years of employment.
• Current DE RN license or compact state license.
• Minimum 2 years of recent HomeHealth RN experience.
• Prior experience in Home Healthcare and Med-Surgical settings preferred.
