Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse to join our HomeHealth team at ChristianaCare. As a Full-Time RN, you will provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and ensure the well-being of our patients.
Responsibilities
- Administer skilled nursing care and treatments to geriatric patients.
- Develop and implement patient-centered care plans.
- Facilitate patient discharge planning.
- Maintain proficiency in IV functions.
Requirements
- BSN required; ADN candidates must obtain BSN within three years.
- Current DE RN license or compact state license.
- Minimum 2 years of HomeHealth RN experience in the last 12 months.
- Previous Home Healthcare and Med-Surgical experience preferred.
