Job Description
We are seeking a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our team at ChristianaCare. As a valued member of our organization, you will provide skilled nursing care and treatment to geriatric patients in the community, with a focus on patient-centered care and discharge planning.
Responsibilities
- Administer skilled nursing care and prescribed treatments to geriatric patients in the community setting.
- Develop and implement goal-oriented, patient-centered care plans within resource constraints.
- Prepare patients for discharge and ensure continuity of care.
- Proficiency in IV procedures is necessary or willingness to undergo training.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of employment.
- Valid DE RN license or compact state license.
- Minimum 2 years of recent Home Health RN experience.
- Prior experience in Home Healthcare and Med-Surgical settings preferred.

Compensation
Pay: $32.00 - $44.00 per hour. Expected hours: 20 - 40 per week.
