Job Description
ChristianaCare HomeHealth is seeking a Full Time RN to join our team. The successful candidate will provide skilled nursing care to geriatric patients in the community. This role involves developing and implementing patient-centered care plans and coordinating patient discharge. The RN will work day shift hours, one weekend a month, two holidays a year, and two on-call shifts a month in the Newark, DE area.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Develop and implement patient-centered care plans.
- Coordinate patient discharge planning.
- Must be IV proficient or willing to train and perform IV functions.
Requirements
- BSN required; ADN candidates to obtain BSN within three years of employment.
- Current DE RN license or compact state license.
- 2 years of recent HomeHealth RN experience.
- Previous Home Healthcare and Med-Surgical experience preferred.
