Job Description
We are currently seeking a dedicated Registered Nurse (RN) Field Nurse for Home Care services to join our team at ChristianaCare. As a member of our HomeHealth team, you will play a crucial role in providing skilled nursing care and treatment to geriatric patients in the community. Our state-of-the-art Electronic Health Record system, comprehensive OASIS-E training, and supportive leadership team will empower you to excel in your career.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Develop and implement goal-oriented, patient-centered care plans.
- Ensure proficiency in IV functions within 90 days of hire.
- Collaborate with the healthcare team to discharge patients effectively.
Requirements
- BSN required; ADN candidates must obtain a BSN within three years of start date.
- Current DE RN license or compact state license.
- Minimum of 2 years of HomeHealth RN experience within the last 12 months.
- Prior experience in Home Healthcare and Med-Surgical settings preferred.
- IV proficiency or willingness to train in IV functions.
