Job Description
We are seeking a dedicated Registered Nurse (RN) Field Nurse for Home Care to join our team at ChristianaCare. In this role, you will provide skilled nursing care to geriatric patients in the community, develop care plans, and ensure patient-centered treatment.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop goal-oriented and patient-centered care plans.
- Prepare patients for discharge.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- 2 years of recent HomeHealth RN experience.
- Previous experience in Home Healthcare and Med-Surgical preferred.
