Job Description
ChristianaCare HomeHealth is looking for a dedicated Registered Nurse (RN) Field Nurse to join our team. In this role, you will provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and ensure the well-being of our patients. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop goal-oriented and patient-centered care plans.
- Prepare patients for discharge and ensure continuity of care.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of employment.
- Current DE RN license or compact state license.
- Minimum 2 years of recent HomeHealth RN experience.
- Proficiency in IV functions or willing to train.
