Atrium Health is seeking a passionate Registered Nurse (RN) for a Home Health position in Charlotte, North Carolina. As a member of our team, you will be responsible for providing high-quality care to adult and geriatric patients in the comfort of their homes.
Job Summary
The Registered Nurse will serve as a case manager, overseeing post-acute services in a home setting, providing direct patient care, and coordinating with a multidisciplinary team to ensure comprehensive care. Responsibilities include developing and implementing care plans, performing treatments, and evaluating patient progress towards established goals.
Essential Functions
- Manage assigned patient caseload for timely care and documentation.
- Collect and analyze patient data for effective resolution of healthcare issues.
- Prioritize patient health and welfare, ensuring high-quality outcomes.
- Coordinate care plans with clinical team members for optimal patient results.
- Perform treatments and procedures in compliance with established protocols.
Requirements
- Graduate from an accredited School of Nursing; BSN preferred.
- Current RN license or temporary license as per state requirements.
- BLS certification required.
- Valid driver's license.
