Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate medical conditions and functional capabilities.
• Provide direct patient care and create rehabilitation plans tailored to individual needs.
• Ensure accurate EMR documentation and compliance with protocols.
• Collaborate with patients to achieve optimal rehabilitation outcomes.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold a valid and unencumbered Physical Therapy license in New York.
• Proficiency in EMR documentation preferred.
• Current CPR Certification.
