Paradigm Rehab is currently seeking a dedicated Rehabilitation Therapist to join our compassionate team. As a Rehabilitation Therapist, you will play a crucial role in helping individuals reconnect with their environment to enhance their quality of life.

Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and assess rehabilitation potential.
• Provide direct patient care aimed at improving functional capabilities and limitations.
• Document patient progress and treatment plans using EMR systems.
• Ensure patient safety and well-being during therapy sessions.

Requirements

• Graduation from an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• Proficiency in conducting PT assessments.
• CPR Certification.
